It's the game that's changing and with it the players. While 10 years ago you'd have someone like Roy Keane playing on injections and a handful of painkillers, today there's the concussion rule and plenty other measures to assure the players' safety. Also the Yeovil game showed just how different and more physical the approach in lower devisions is.
But anyway, I'm pretty sure our players aren't going "I have an ouchie, I don't wanna play today" because there's no player that wants to sit games out. So it's gotta be a decision based on the medical staff's opinion.

It looks to me that some players are perfectly happy to sit on the bench while getting (not earning) their salaries, especially those out of form and confidence and the ones with the attitude problem, like known troublemakers and other different kind of gits. Ando and Evans looks exactly like that to me but I could be imagining. However, I'm pretty sure that there's some who are not bothered too much whether they'll play or not. It's just that new rules and too much precaution are made players to be a real milksops. You've mentioned Keane. He did play on injections but he also wasn't a wimp like majority of the modern footballers. I'm not saying go out and kill each other but show some guts, grit your teeth and continue.

I think it all goes down to your idea what footballers should be like. Those type of macho, overly manly men aren't produced anymore and the quicker you stop comparing then vs now, the better. These players work really hard to get through all the levels so they can reach the top, so I sincerely doubt they're not bothered whether they'll play or not. As for the troublemakers you only need to look at how Ravel Morrison is doing.
